Student Forms and Fees

We will be having an all school registration for 4k-12 on August 2nd and 7th 2012 from 8:00am-6:00pm at the Baraboo High School.  Please use this page to find all of your children(s) forms.  It would be helpful if you print out/fill out all forms prior to registration.  You can also pay all fees online using the Infinite Campus Portal prior to registration.  Specific fees are listed below.  Please contact 355-3940 if you have any questions regarding the registration process.

2012-13 Fees

We encourage you to pay all of our fees online using your district Infinite Campus Parent Portal.  If you don't have a portal accout, please contact your local school.

Elementary Fees
Grades K-5:     $15.00

Middle School Fees
Grades 6-8:     $25.00 Registration Fees
Grades 6-8:     $4.50   Pool Use Fee
Grade 6:          $2.50 Family & Consumer Science
Grades 6-8:     $13.00 Physical Education Uniforms
Grade 8:          $5.00 Technology Education
Grade 8:          $5.50 Foods Class

Grades 9-12:    $25.00
Driver Education $300
Parking Permit $20 per semester OR $35 per year
Sports $60 with maximum of $200
(Hockey and Swimming don't count toward the $200 "Waiver" of the fee is available for those unable to pay.)
